This policy explains how information is collected and handled through this website and its contact options. “I” refers to Dr. Lindsay Wakayama acting on behalf of the professional corporation identified above.

My Notice of Privacy Practices separately explains how protected health information is handled and describes your patient privacy rights. Information you provide when requesting health care may also qualify as protected health information. This website policy does not replace or limit those protections.

Information collected

Information you provide. When you contact the practice, I receive the information you submit. This may include your name, date of birth, email address, telephone number, state, optional demographic information, service interests, scheduling preferences, and message. The linked SimplePractice inquiry form collects identifying and contact information and allows you to provide additional details about your request.

Website information. Squarespace, the website’s hosting provider, may collect technical and usage information, including your IP address, browser and device information, pages visited, referring website, and interactions with the site. Restricting non-essential cookies does not eliminate all technical or usage-data collection.

How information is used

I use information to respond to inquiries, discuss services, coordinate consultations and scheduling, provide Good Faith Estimates, and manage related practice communications. Technical information may be used to operate, maintain, secure, troubleshoot, and understand use of the website.

Please provide only the information needed to address your inquiry. Do not include payment-card details, Social Security numbers, or detailed clinical records in an initial message. When additional information is needed, I will explain how to provide it through an appropriate channel.

Submitting an inquiry does not establish a psychologist-patient relationship. Messages are not monitored continuously and should not be used for urgent help.

Service providers and information sharing

This website is hosted by Squarespace. Inquiries submitted through the linked contact form are received and managed through SimplePractice. I use Google Workspace to send, receive, and store practice email.

These providers process information as needed to deliver their respective services. Information may also be disclosed to professional advisers, public authorities, or other recipients when legally permitted or required.

When information is protected by HIPAA or California health-privacy law, those protections govern its use and disclosure, including applicable safeguards and business associate agreement requirements. This policy does not authorize disclosures that would otherwise be prohibited.

Cookies, tracking, and external services

Squarespace’s non-essential cookies are restricted on this website. Essential cookies may still support functionality and security, and basic technical and usage information may still be collected. I have not enabled Google Analytics or Meta/Facebook Pixel on this website. You can manage or delete cookies through your browser settings, although blocking some cookies may affect website features.

Browser privacy signals. The restriction on non-essential Squarespace cookies applies regardless of your browser’s “Do Not Track” setting. For information it processes under its own privacy policy, Squarespace states that it does not respond to Do Not Track signals but does respond to Global Privacy Control signals in California. These are different browser privacy mechanisms. Details are available in Squarespace’s Privacy Policy.

Third-party services may independently collect information about your activity over time and across websites when you use their services. When you follow a link to SimplePractice or another external website, that provider’s privacy and cookie practices apply to its service. Those policies do not replace my responsibilities for information handled on behalf of this practice.

Security and retention

I use reasonable safeguards appropriate to the information being handled. However, no internet transmission or electronic storage system can be guaranteed completely secure.

Information is retained for as long as reasonably necessary to respond to requests, manage practice operations, and meet applicable legal and professional obligations. Information that becomes part of a clinical or billing record is subject to the retention requirements applicable to those records.

Your choices and requests

You may choose not to submit personal information, although this may limit my ability to respond to your request.

To request access to, correction of, or deletion of information you have submitted, please contact me using the contact option below. I may need to verify your identity before acting on a request. Some information must be retained to meet legal, clinical, or recordkeeping obligations. Requests concerning clinical records are addressed under my Notice of Privacy Practices and applicable law.

Changes to this policy

Updates will be posted on this page with a revised effective date. Material changes will also be identified through a prominent website notice or direct notification, as appropriate. Any additional notice or consent required by law will be provided or obtained.
